Is My Ferritin Normal? What High and Low Ferritin Mean

Short answer: ferritin reflects your stored iron, but it's also an "acute phase reactant" — meaning it rises with inflammation independent of your actual iron status. A ferritin result needs both of those lenses to interpret correctly, which is why "normal" depends on more than the number alone.

What ferritin measures

Ferritin is the protein your body uses to store iron. A blood ferritin test is used as a proxy for total-body iron stores — low ferritin generally means low iron reserves, and very low ferritin is one of the earliest signs of iron deficiency, often showing up before red blood cell counts change.

Reference ranges vary meaningfully by lab and by sex — commonly cited ranges run roughly 20–250 ng/mL for men and 20–150 ng/mL for women, though menstruating women often run toward the lower end of that range normally. Always use the specific reference range printed on your own lab report, since methodology differs between labs.

Why high ferritin isn't automatically "too much iron"

This is the part people most often misread: because ferritin rises during inflammation, illness, heavy alcohol intake, or metabolic dysfunction, an elevated result doesn't necessarily mean iron overload. It can equally mean your body is responding to something inflammatory that has nothing to do with iron at all. That's the reason ferritin is often interpreted alongside other iron markers (like serum iron, TIBC, or transferrin saturation) and inflammation markers rather than in isolation.

What commonly moves ferritin

  • Low: heavy menstrual blood loss, restrictive diets, endurance training, undiagnosed GI blood loss.
  • High: inflammation or infection, high alcohol intake, metabolic dysfunction, or true iron overload conditions.
