What Is the Omega-3 Index?

Short answer: the Omega-3 Index measures what percentage of the fatty acids in your red blood cell membranes are EPA and DHA — the two long-chain omega-3 fats found in fatty fish and fish oil. It's reported as a percentage, and it reflects your omega-3 status over roughly the past 3–4 months, not just what you ate recently.

Why red blood cell membranes, specifically

Unlike a snapshot of omega-3s currently circulating in your blood plasma (which shifts quickly based on your last few meals), red blood cells live for about 3 months, and their membrane composition reflects your average intake over that whole lifespan. That makes the Omega-3 Index a more stable, longer-window measurement than testing blood omega-3 levels directly.

Why it's tracked as a cardiovascular and inflammation marker

EPA and DHA are incorporated into cell membranes throughout the body, including in the heart and blood vessels, and are precursors to anti-inflammatory signaling molecules. Higher Omega-3 Index values are associated with better cardiovascular outcomes in observational research, which is why it's commonly grouped with inflammation and cardiovascular markers rather than treated as a standalone nutrition metric.

What moves it

  • Fatty fish intake (salmon, sardines, mackerel) — the most direct dietary lever.
  • Fish oil or algae-based EPA/DHA supplementation, at a high enough dose taken consistently.
  • Individual absorption and metabolism, which vary meaningfully between people even at the same intake level.

Because red blood cells turn over slowly, changes from a new supplement or diet shift typically take 8–12 weeks to show up meaningfully in a retest — testing again after only a few days or weeks won't reflect much change yet.
