Why Did My HOMA-IR Go Up?

Short answer: HOMA-IR is a calculated estimate of insulin resistance, derived from your fasting glucose and fasting insulin together — not measured directly. If it went up, either your fasting glucose, your fasting insulin, or both moved in a direction that suggests your body needs more insulin to manage the same blood sugar level.
How HOMA-IR is calculated
HOMA-IR = (fasting glucose × fasting insulin) ÷ 405 (when glucose is measured in mg/dL and insulin in µIU/mL). Because it's a product of two values, a rising HOMA-IR score doesn't tell you which one moved — you have to look at fasting glucose and fasting insulin individually to see what actually changed.
A commonly cited optimal target is roughly ≤1.2 — tighter than the broader "not yet insulin resistant" thresholds sometimes cited in research (often placed closer to 2.5–3.0), since an optimal target aims lower than merely avoiding a clinical label.
What commonly drives HOMA-IR higher
- Poor sleep, even short-term — a few nights of restricted sleep measurably reduces insulin sensitivity.
- Reduced physical activity or a period of lower step counts / less resistance training.
- Diet pattern shifts — particularly more refined carbohydrates or late-night eating.
- Elevated stress load, which raises cortisol and can blunt insulin sensitivity.
- Weight change, particularly increases in visceral fat.
- Not fasting correctly before the blood draw — this alone can shift the result meaningfully.
What to do with one elevated result
Because both inputs are sensitive to short-term behavior (sleep the night before, recent meals, stress), one elevated HOMA-IR is a prompt to look at the trend over your next few draws and the weeks around them — not a standalone conclusion. If it's paired with steady week-over-week movement in the same direction, that's a stronger signal than one isolated reading.
